Cancellation Policy
Lifestyle Preceptor Administrative Policy Jeffrey A. Hirschfield, M.D., P.A. Location: 6705 38th Avenue North, Suite A, St. Petersburg, FL Contract Coordination Lifestyle Preceptor will coordinate directly with the institution to obtain a fully executed contract. All terms, including administrative fees, will be governed by this document. Administrative Fee Structure • A non-refundable administration fee will be included, based on institutional requirements. • Upon execution of the contract by institution, preceptor, and student: • 50% of the administration fee is due by check at the time of contract execution between preceptor and student. • Remaining 50% is due no later than 60 days prior to the preceptorship start date. Start Date Policy • Requested changes to the initial start date are not guaranteed. • At our sole discretion, we may approve a revised start date, not to exceed four (4) months from the original start date.
